ANDROID: debug_kinfo: add module_memory offset

In commit ac3b4328 ("module: replace module_layout with
module_memory"), the way module memory layouts are allocated and
accessed has been changed. Thus we need to add a new member to debug_kinfo structure, module_memory offset, used for original design.
