Revert "scsi: replace blk_mq_pci_map_queues with blk_mq_map_hw_queues"

This reverts commit 5e8438fd which is
commit bd326a5a upstream.

It breaks the Android kernel abi and can be brought back in the future
in an abi-safe way if it is really needed.
