usb: gadget: uvc: replay the downstream changes

Now that we have multi instance support patches in, replay
the previously reverted downstream patches. These patches
are reworked as there are many changes to uvc configfs
interface. This exercise also helps in porting these
downstream patches to latest kernels when required.
