msm: subsystem_restart: Remove support for RESET_SUBSYS_MIXED 



RESET_SUBSYS_MIXED was a temporary implementation for
the 8660 to allow the modem(s) to restart independently
of the lpass and still allow the modem(s) to be restarted
when the lpass crashed.

This implementation is no longer necessary. Remove it.
