bluetooth: qca: Add Bluetooth support for QCA6391 chipset 



QCA6391 is functionally similar to QCA6390, hence just reuse the same
settings to get Bluetooth working. This assumes that the chipset is
properly powered ON before this driver probes.

Additonal improvement in future would be to add regulator and BT_EN gpio
support in this driver.
