Fix to resume held call and answer waiting call 

While answering waiting call update the waiting call as the call
to be resumed so that the held call is automatically resumed when
waiting call terminates.
When the HOLD request on the ACTIVE call fails due to the call being
terminated answer the ringing call before the active call session
is closed
