SystemUI: Upgrade the logic of 5G NSA icons 

Modem determines which 5G icon to display, so SystemUI only uses
queryNrIconType and onNrIconType to display 5G NSA icons.

Remove the deprecated logic which depends on dcnr, upperLayerIndInfo,
plmn and etc.

Rework the unit test.
