BT HFP AG: Remove SCO devices during abnormal BT off

When BT process is killed or restarted, there is a chance
for failure while removing SCO device from mConnectedDevices
hash map. This results in SCO devices selection failure until
phone is rebooted. Attempt to remove SCO device with other SCO
devices i.e DEVICE_OUT_BLUETOOTH_SCO_HEADSET and
DEVICE_OUT_BLUETOOTH_SCO_CARKIT before bailing out.
