Correct Fonic Germany (262/07) MVNO definition to use IMSI match

Changes for BT Mobile UK new MVNO agreements:
    Correct 234/15 to use GID B3 match and PAP authentication
    Add 234/30 entries for business and consumer SIMs (GID B3/C3)
    Add 234/08 entry for BT One Phone SIM when in UK (GID B2)
    Add 234/30 entry for BT One Phone SIM when roaming (GID B2)
